Below you will find the link to the new online form where the DCYF Regionals Contracts Team will collect data for our Contracts System and Service Directories that DCYF Case Workers use. If you have questions or need assistance, please reach out to the Contracts Specialist from whom you received your renewal email.
Contracted Services Staff List
Please complete a staff list for each Contracted service you are renewing and upload it to the online form.
Note: when you open the staff list a dialog box may pop open, please click the “enable content” button to enter information into the new staff list.
