Child Abuse and Neglect (CA/N) History Checks

The Department of Children, Youth & Families (DCYF) maintains records of founded findings of child abuse and neglect. This information is limited to substantiated findings, does not include investigations determined as inconclusive or unfounded, and requires the subject individual’s authorization to release this information.

The department’s CA/N History Checks Unit has customized forms and processes for a variety of purposes. Review the options to determine which apply to your needs:

Child Protective Services (CPS) Investigators Outside of Washington

  • No form required

Public child welfare agencies may submit a CA/N history check request as part of an open CPS or child welfare investigation. Requests must be submitted using a state agency's letter head.

Provide the following information in your request: 

  • Subject individual’s full name and other names used
  • Subject individual’s date of birth
  • Full names and dates of births of alleged victims associated with the subject individual and their relationship. Requests without this information will be rejected.
  • Social security numbers for subject individual and related persons, if available.

  Requests that do not include alleged victims’ information will be returned as unable to complete.

Results are returned to the original requestor by encrypted e-mail.

All Other Out-of-State Requests

Purposes include foster home licensing, relative placement, adoption, employment, volunteers or interns in settings requiring child abuse and neglect history checks. A $20.00 fee is required for each individual's CA/N request. When submitting multiple requests, one check may cover the total cost.

  • Submit one completed form for each individual.
  • Forms must be typed (electronically filled-out) for legibility.
  • Forms must be dated and signed by both the requestor and subject of the search.
  • Handwritten or incomplete forms that are returned for correction may delay results.
  • Include a check in the amount of $20 per individual request, made payable to DCYF and mail completed requests to:

For questions, call 1-800-998-3878. Once the payments and requests are received, results are returned to the original requestor by encrypted e-mail.

  Fees are not required for a Washington dependent child placed outside of Washington through an Interstate Compact for the Placement of Children, (ICPC). The requesting ICPC state must provide the Washington DCYF case number and email the completed form to CANhistorychecks@dcyf.wa.gov.

DCYF 23-032 form should only be used by requestors in Washington state seeking CA/N information for current Washington state residents. This includes Washington state residents who are attending school out of the state of Washington. Purposes include foster home licensing, relative placement, adoption, employment, volunteers or interns in settings requiring child abuse and neglect history checks.
