Adolescent Substance Use Assessment (ASUA)

The ASUA assesses youth in the six American Society of Addiction Medicine Dimensions, providing Substance Use Disorder diagnosis according to Diagnostic and Statistical Manual-5-TR.  This ASUA also provides treatment recommendations and captures self-reported drug use history.

Integrated Developmental Evaluation and Assessment (IDEA):

The IDEA is a 130-item Risk, Needs and Responsivity assessment comprised of both static and dynamic factors designed to identify youth’s risk and needs upon entry to Juvenile Rehabilitation, and throughout their commitment, with reassessment every 90 days. The IDEA is utilized in treatment planning, and identification and access to services including Community Transition Services as legislated by HB 1186.

Life Events Checklist for DSM-5 (LEC-5)

The LEC-5 is a self-report measure designed to screen for potentially traumatic events in a respondent's lifetime.

Patient Health Questionnaire– 9 (PHQ9) Anxiety Screen

PHQ-9 sample

The PHQ-9 is designed for use with adults in assessing and monitoring depression severity and consists of nine items that focus on the diagnostic criteria for major depressive disorder.

The PHQ9 informs Dialectical Behavioral Treatment level of care, and professional clinical care, and progress (measurement-based care).

Posttraumatic Stress Disorder Checklist for DSM-5 (PCL-5)

The PCL-5 is a 20-item adult self-report rating scale for assessing PTSD symptoms over the past month that maps directly to DSM-5 criteria.

Pre-Placement Screen

The Pre-Placement Screen is used by Regional staff with all young people adjudicated or committed to DCYF/Juvenile Rehabilitation to determine initial secure facility placement based on age, gender, behavior, escape history, security risk, medical needs.

Risk Assessment Recidivism

This assessment is used on all young people adjudicated or committed to DCYF/ Juvenile Rehabilitation to determine initial risk level, release from incarceration within the sentencing range, parole eligibility, and community facility placement suitability.

Sex Offender Supervision Screen (SOSS)

This tool is used for youth who have sexually offended who are on parole. It determines whether to extend their parole past 24 months (up to 90 days at a time) to 36 months as allowed in statute RCW 13.40.210 (3)(a). It is completed by the regional Sex Offender Treatment Coordinator in collaboration with a youth’s community sex offender treatment provider.

Sexual Orientation, Gender Identity, and Expression (SOGIE)

The SOGIE is administered within the first 72 hours of residential obligation. This tool is meant to identify an individual who may be particularly vulnerable to abuse or harassment due to gender-related concerns.

Sexually Aggressive Vulnerable Youth (SAVY)

The SAVY is used in residential settings for room placement and to assesses if a youth may be particularly vulnerable to sexual aggression or prone to be sexually aggressive.

Suicide Self Harm Screen (SSS)

The SSS is administered at intake to an institution or community facility; when there is evidence of increased risk factors or warning signs; or, when a reduction or removal from Suicide Precaution Level (SPL) may be warranted due to reduced risk or increased protective factors.

Supervisory Case Review (SCR)

This assessment is used for young people with a parole aftercare obligation. The SCR is completed when requesting a Functional Family Parole phase change. The SCR is also used for young people on parole for a sex offense at periodic intervals to adjust their time on parole.

Washington state sex offender risk level classification Tool (WSSORLC)

This WSSORLC provides a risk level classification recommendation to law enforcement for community notification purposes.  The End of Sentence Review Committee (ESRC) outlined in RCW 72.09.345 requires review of all juveniles with a registerable sex offense, whether committed to JR or not, to make a level recommendation to law enforcement.

Child and Adolescent Trauma Screen (CATS)

The CATS is a DSM-5 based measure that includes 15 potentially traumatic events or series of events, 20 posttraumatic stress symptoms, and 5 impairment items.
